The Adult Day Health Center is located at 2272 Pyramid Way Sacramento CA 95821. Facility is managed by Collin Gobounre RN and a host of consummate professionals and associations

We have established a sound fiscal system to maintain financial stability and self-sufficiency, ensuring our long-term sustainability.

Living Well focus is on frail and elderly people and other impaired adults, ages 18 and older who meet any of the following criteria:
The Living Well facility has a multidisciplinary team of health professionals that conduct a comprehensive assessment of each potential participant to determine which ADHC services meet the individual specific health and social needs. Each participant is provided with an individual plan of care which may include any of the following required services:
Objectives of the program are: To restore or maintain optimal capacity for self-care by frail elderly people and other physically or mentally impaired adults
